MEDIAL EPICONDYLITIS TREATMENT AT MAYWOOD PHYSICAL THERAPY & REHAB CENTER WITH THE USE OF SHOCKWAVE ESWT AND STA (Stabilizing Therapeutic Activation) System© / ECCENTRIC EXERCISES

Treatment consisted of 5 sessions of Extracorporeal Shockwave Therapy (ESWT) at 2,000 shocks at 10HZ at an Energy ranging from 3.0-3.5 Bar. The treatment plan also consisted of proper STA/ Eccentric Exercises as outlined below.

HISTORY :

Patient is a 64-year-old male with complains of chronic pain and tenderness at medial right elbow after years of manual labor from work as a mechanic. Patient's pain began in October 2016 and has become worse and intolerable since. Patient reports he received a cortisone shot on 11/28/16- with little and/or momentary effect. Patient has trouble heavy lifting, performing house work, performing work duties, and occasional episodes of pain withactivities of daily living.

CURRENT FUNCTIONAL LIMITATIONS:

  • Carrying, Moving & Handling Objects: Difficult to perform secondary to pain Pain Location: Right Medial Elbow
  • Pain Scale: Worst: 8/10
  • Pain Description: Sharp
Aggravating Factors: Work
General Health: Good
Medical History: Hight Blood Pressure, High Cholesterol, Arthritis, Measles
Diagnostic Testing/Imaging: Patient reports receiving X-Ray (-)
Complicating Factors: Works Long Hours
Mental Status/Cognitive Function Appears Impaired? No
Current Medications: Benicar 20 mL, Atorvastatin 20 mL
Patient Goals: To be able to work pain free.

ECCENTRIC EXERCISE TREATMENT PLAN:

  • Eccentric Right Wrist Flexion Protocol 5x15
  • 10" lowering with 5# DB
  • Active Right Wrist Flexion 4# 3x10 5" Tempo
  • TheraBand RightEccentric Wrist Flexion 3x15
  • TheraBand Concentric Flexion/Extension 3x15-20
  • TheraBand RightPronation/Supination 3x15-20
  • Eccentric Supination 2.5# ankle weight + wooden bar 50-75 reps
  • Eccentric Supination/wrist flexion
  • Manually Resisted Exercise + wooden bar
  • Right Wrist Flexion/Ext Stretch 3x30" each

PATIENT PROBLEMS:

  • Posture
  • Pain in right elbow
  • Stiffness in right elbow with work activities
  • Decreased Elbow Pronation
  • Manual Muscle Test - Decrease Wrist Flexion Manual Muscle Test - Decrease RUJ joint mobility - Increase pain in RightMedial Elbow

GOALS:

  • (3 Weeks) |Patient will have 4/10 pain at all times in order to perform daily Activities of Daily Living with less difficulty.
  • (3 Weeks) | Patient will improve Right wrist flexion Manual Muscle Test to 4/5 in order to lift heavy items with less difficulty.

LONG TERM GOALS:

  • (6 Weeks) | Patient will improve all Right elbow and wrist Manual Muscle Test to 5/5 in order to perform work duties withoutdifficulty.
  • (6 Weeks) | Patient will have 0/10 pain at all times in Right elbow to perform house work without difficulty. | 3: (6 Weeks) | Patient will be (I) with written Home Exercise Program.

CONCLUSION:

END OF TREATMENT SESSIONS: Patient. was able to complete treatmentsession w/ithout of pain or difficulty. Patient. able to perform therex for improved upper extremity function w/ithgood tolerance. Patientwith good tolerance of manual interventions.

GOALS MET: 95% | Patient will improve all rightelbow and wrist MMT to 5/5 in order to perform work duties without difficulty. | 5% | Patient has 0/10 pain at all times in rightelbow to perform house work withouto difficulty.
